A "Guest Player" is defined as any player that is NOT listed on your OFFICIAL ROSTER.  Only players on your official roster are covered under the mandatory insurance.  By filling out the MSYSA (or your State/Country equivalent) guest player form will insure your guest players are covered during the tournament.  Click the link below to the MSYSA Guest Player form.  The cost is $10 per player and please make sure bring the completed form (which will be emailed to you) to the Team Check-In tent at the beginning of the tournament.

Each player is required by the MSYSA to have a signed Medical Release form.  Out of state/country teams should aquire these forms from their state or country soccer association.  For Michigan teams, you can click the link below for the MSYSA Medical Release form.
